- Mobile Outreach Case Manager
Description
Youth and Young Adult (YYA) services support homeless individuals between 18-24 years of age who are experiencing homelessness. This population faces numerous challenges, including family estrangement, unemployment, homelessness, and/or behavioral/mental health conditions. The focus of this service is to help young adults find appropriate housing and provide supportive services along with case management services.
The Outreach Case Manager will provide the resources and support needed to improve the overall health, social, and economic outcomes of the YYA. The Outreach Case Manager is responsible for promoting the program throughout Bristol County, targeting YYAs who are at risk of becoming homeless or are homeless and in need of housing stability; they will conduct community outreach to develop resources that support this population. The Outreach Case Manager will be part of a multi-disciplinary team that provides case management, housing referral, financial counseling and benefits services.
Requirements
BA/BS in Human Services preferred, experience in human services
Experience working with youth and young adults or homeless individuals
Experience in providing housing stability related Case Management
Excellent organizational and communication skills
Computer skills with the ability to perform daily tasks within HMIS operating systems, Outlook, Microsoft Word, and Excel
A reliable, insured vehicle and valid driver’s license required
Bi-lingual helpful
