Description

We’re looking for an LPN clinical nurse coordinator to support care navigation across the continuum of cancer by facilitating guideline-aligned treatment, symptom tracking, and member re-education in partnership with RNs, providers, and an interdisciplinary team. This is a full-time, remote position. 

What You'll Do:

• Maintain regular contact with assigned oncology members via phone, text, email, and video calls to provide follow-up and support
• Assist and coordinate with the RN by communicating with providers and pharmacies and facilitating referrals to third-party infusion services
• Provide education and guidance on treatment logistics, medication access, and community or program resources, reinforcing physician-directed care
• Identify, coordinate, and facilitate referrals to infusion services, clinical resources, or supportive care programs

Requirements

What We're Looking For:

• Licensure: Active LPN license
• Experience: 4+ years of LPN/LVN nursing experience in a related role, including clinical experience in oncology, infusions, critical care, care navigation and complex case management
• Must speak, write, and communicate fluently in English and Spanish
• Location: Based in the Pacific, Arizona, or Mountain time zone required
• Workspace: Quiet, secure location with reliable high-speed internet (U.S.-based only)

Schedule:

• Full-time, remote-position
• Monday–Friday during normal business hours with occasional evenings and weekends as needed

• Compensation: $40,000-$60,000 annual salary
• Benefits: Health, Dental, Vision, PTO, and 401K
