Full-time (40 hrs.) RN Case Manager Position w/ Benefits
The Visiting Nurse Home Care & Hospice of Carroll County is seeking a compassionate and driven RN Case Manager to provide the highestquality, oneonone care for patients in their homes. In this missiondriven role, you’ll use your clinical skills and your heart for service to deliver communityfocused care that helps patients remain safe, comfortable, and independent during some of their most vulnerable moments.
Job Summary
Provide admissions, case management, and followup nursing visits.
Develop and update individualized plans of care with an interdisciplinary team.
Deliver skilled nursing care (assessments, meds, wound/IV care, education).
Build trusting relationships and educate patients and families to safely manage care at home.
Collaborate with physicians and other clinicians to coordinate patientcentered care.
Advocate for patient needs while maintaining excellent, compliant documentation.
Requirements
Qualifications and Skills
Must be a registered nurse in New Hampshire or Maine
Strong organizational and communication skills
Reliable transportation
Valid driver’s license and auto insurance (minimum ins. limits of $100,000/$300,000)
Clean Background checks will be required
Availability to rotate on weekends and holidays on a limited basis each year
Prior home health experience is preferable
