Description

Job Purpose: 
We are looking for a highly professional, dependable and service-oriented individual who is passionate about internal medicine and providing excellent care to patients.

Responsibilities:

  • Assess patients’ needs and educate them on medical processes and procedures.
  • Effectively triage and manage acute and chronic medical condition.
  • Through discussions with the physicians and patients, assist in communicating an overall care plan to patients.
  • Make and answer phone calls, both pre and post treatment, from patients, referring providers and outside facilities (e.g., hospitals, rehabilitation centers, physician offices).
  • Work collaboratively with physicians, assisted living facilities, and hospitals to assist in the enrollment of patients into appropriate programs.
  • Consult with physicians on medications and manage the process, ensuring vaccines and medication supplies are ordered and stocked as appropriate.
  • Coordinate medical records – document symptoms, findings, treatment plan., and other information necessary for patients’ charts.
  • Administer vaccines to patients and perform phlebotomy.
  • Authorize prescription drug refills as directed by physician and ensure prescriptions are called in to pharmacies in a timely manner.
  • Demonstrate strong commitment to outstanding service and care to patients.
  • Pull together prior authorization for medical procedures, as needed.
  • Along with colleagues, ensure exam rooms are prepared for incoming patients – organizing instruments, supplies and equipment.


Requirements

Education and Experience:

  • Bachelor’s Degree
  • CT licenses

Required skills and experience:

  • Exceptional interpersonal skills.
  • Experience in a service business.
  • Strong leadership skills; forward thinker.
  • Strong knowledge of medications and injection procedures.
  • Phlebotomy
  • Ability to work well under stress and remain patient focused.
  • Strong team player.
  • Highly dependable.
