Summary

Job Description

HOURS: Full Time (1.0 FTE; 40hrs/wk) Monday-Friday no weekends and no holidays. Work hours can be flexible after probationary period, with primarily working hours being between 6 AM and 5 PM. Remote work available.

POSITION SUMMARY: The Clinical Integration Specialist (CIS) is incorporated into the clinical team and is responsible for the documentation integrity of the medical record for accurate severity of illness. The CIS is responsible for improving the overall quality of the medical record, including accurate diagnoses, accurate capture and documentation of core measures, and prevention of patient safety indicators. The CIS communicates relevant findings to treating physicians, may perform direct patient evaluation when appropriate, and assists with determination of medical necessity.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Initiates and performs concurrent documentation reviews of selected inpatient records to clarify conditions/diagnoses and procedures where inadequate or conflicting documentation exists. 
  • Facilitates documentation improvement methodology to support appropriate clinical coding for reimbursement and capture of the severity of patient diseases (case mix) treated at LGH.  In addition, ensures completeness of clinical information used for reporting physician and hospital outcomes.
  • Accurate and timely record review.
  • Recognize opportunities for documentation improvement.
  • Formulate clinically credible documentation clarifications.
  • Timely follow up on all cases especially those with clinical documentation clarifications.
  • Communicates with HIM staff and resolves discrepancies.
  • Interaction with physicians to facilitate complete and accurate documentation within the patient health record.  Serves as a resource for physicians to help link ICD-9-CM coding guidelines and medical terminology to improve accuracy of final code assignment.
  • Provides medical necessity review as appropriate.
  • Educates internal customers on clinical documentation opportunities.  Develops and conducts ongoing CDI education for new staff, coders, physicians, residents, nursing, and allied health professionals.  Develops education program materials including online materials, handouts, overheads, PowerPoint presentations, etc.
  • Participation in Task Force meetings.
  • Identifies opportunities for CDI operational improvements.
  • Utilizes CDI review worksheet and concurrent query as tools to capture improved clinical documentation.
  • Utilizes hospital information technology software to collect, track, and report outcomes and the effectiveness of the data.
  • Utilizes software as a resource in ensuring accurate documentation.  Tracks responses and trends completion of concurrent queries.
  • Monitors and evaluates effectiveness of concurrent coding outcomes at designated intervals.  Report concurrent coding outcomes to hospital departments and committees at designated intervals.  Communicates information effectively. 

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

  • Demonstrates responsibility for personal development by participating in continuing education offerings.  Maintains competence related to MS-DRG assignment, documentation requirements, and coding guidelines.
  • Maintains accurate records of review activities.
  • Other duties as assigned.

Requirements

MINIMUM REQUIRED QUALIFICATIONS:

  • Current licensure as a Registered Professional Nurse, issued by the PA State Board of Nursing
  • Bachelor of Science degree
  • Five (5) years experience in nursing or other clinical area.
  • Effective interpersonal skills in order to effectively interact with all levels of hospital personnel both verbal and written. 
  • Excellent organizational and prioritization skills.
  • Must have excellent analytical skills and be proficient with a computer and Microsoft Office programs.
  • Experience in a similar CDI capacity is preferred.
  • Coding experience is preferred.
