Job Summary
The Director of Nursing (DON) is responsible for providing clinical leadership and oversight for multiple assisted living and memory care homes, ensuring exceptional resident care, regulatory compliance, and operational excellence. This role leads and supports caregiving staff while serving as the primary clinical resource for residents, families, physicians, hospice providers, and the interdisciplinary care team. The DON is committed to delivering compassionate, person-centered care while maintaining compliance with Texas HHSC regulations and company standards.
Essential Responsibilities
Clinical Leadership
Provide clinical oversight for nursing services across multiple residential care homes.
Conduct initial and ongoing resident assessments, including physical, cognitive, emotional, and medication evaluations.
Develop, implement, and update individualized service plans and comprehensive care plans in accordance with HHSC regulations.
Monitor residents for changes in condition and coordinate timely interventions.
Perform direct nursing care as needed, including wound care, injections, assessments, medication review, and delegated nursing procedures.
Coordinate medication management with pharmacies, hospice agencies, and physicians to ensure accuracy and continuity of care.
Ensure residents’ nutritional needs are met through collaboration with dietary providers and caregiving staff.
Staff Leadership & Development
Lead, mentor, supervise, and support caregivers and clinical staff.
Conduct orientation, skills validation, medication administration training, and ongoing education.
Provide nurse delegation and supervision for delegated nursing tasks in accordance with Texas regulations.
Serve as a clinical resource for staff, providing coaching and support as needed.
Regulatory Compliance & Quality Assurance
Maintain compliance with all Texas HHSC regulations and nursing standards.
Ensure accurate and timely clinical documentation.
Monitor incidents, investigate concerns, and implement corrective actions when appropriate.
Participate in state surveys and regulatory inspections, including plans of correction when required.
Conduct routine rounds to evaluate quality of care, resident safety, and regulatory compliance.
Resident & Family Relations
Serve as the primary clinical liaison for residents, families, physicians, hospice providers, home health agencies, and ancillary healthcare partners.
Communicate resident status changes, care updates, physician appointments, medications, and care planning with professionalism and compassion.
Coordinate durable medical equipment (DME), medical supplies, specialty appointments, and clinical services.
Facilitate care conferences and assist families with care planning and transitions.
Operations & Collaboration
Maintain regular communication with each care home to ensure resident needs are addressed promptly.
Collaborate with leadership, marketing, scheduling, therapists, case managers, physicians, hospitals, and community partners to coordinate seamless resident care.
Assist with admissions, assessments, hospital discharges, and clinical transitions as needed.
Support emergency preparedness and ensure staff competency in responding to medical and environmental emergencies.
Demonstrate sound clinical judgment, critical thinking, and effective problem-solving in daily operations.
Qualifications
Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
Current, unrestricted Texas Registered Nurse (RN) license.
Minimum five (5) years of progressive nursing leadership experience in assisted living, memory care, long-term care, home health, or a related senior care setting.
Strong knowledge of Texas Health and Human Services Commission (HHSC) regulations.
Experience supervising, mentoring, and developing clinical staff.
Excellent assessment, clinical decision-making, communication, and organizational skills.
Ability to build trusted relationships with residents, families, physicians, and interdisciplinary care teams.
Demonstrated commitment to compassionate, resident-centered care and continuous quality improvement.
