Position Summary:
The Patient Family Nurse Navigator supports families of neonates in the NICU by providing personalized guidance, advocacy, and coordination of care throughout their hospital experience. Working Monday through Friday, dayshift, this full-time role focuses on empowering families through education, resource connection, and emotional support, ensuring seamless communication and collaboration among care teams. The Navigator will serve as a vital link between families and the medical team, helping to address concerns, facilitate understanding of the care plan, and promote positive healthcare experiences during a critical time.
Key Responsibilities:
- Act as a dedicated advocate and resource for NICU families, guiding them through complex medical information and hospital processes
- Coordinate care needs in collaboration with NICU physicians, nurses, social workers, and other healthcare professionals
- Develop individualized support plans tailored to each family's unique needs and circumstances
- Facilitate clear communication between families and care teams, ensuring families are informed and engaged in decision-making
- Provide emotional support, education, and resources to families during their NICU journey
- Assist with discharge planning and connecting families to community resources and follow-up care
- Document interactions and care coordination activities accurately within the medical record system
Qualifications:
- Requires an Associates of Nursing (ASN).
- Bachelors of Nursing (BSN) preferred.
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
- Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred.
- NICU experience preferred.
- Requires Basic Life Support certification through the AHA.
- Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
- Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.
- Requires knowledge of personal computers using various types of word processing and database software.
- Requires clinical work experience, medical terminology.
- Requires understanding of medical record requirements, regulations and policies.
