ABOUT THIS ROLE
As our full-time Home Health RN or LVN, you will deliver skilled nursing care directly in patients' homes across the Greater Houston area. Your work will include patient assessments, wound care, medication management, IV therapy, and patient and family education. You will collaborate closely with physicians and our care team to develop and carry out individualized care plans that keep patients safe, healthy, and independent in their homes.
WHAT YOU WILL DO
Conduct comprehensive in-home patient assessments and document findings accurately
Administer medications, IV therapy, injections, and treatments per physician orders
Perform skilled wound care, catheter care, and post-surgical monitoring
Educate patients and family caregivers on care plans, medications, and home safety
Complete OASIS assessments (RNs) and 485 plan-of-care documentation
Communicate changes in patient condition promptly to the physician and care team
Maintain accurate, timely electronic health records (EHR) in compliance with Medicare standards
Requirements
WHAT WE ARE LOOKING FOR
- Active, unencumbered Texas RN or LVN license (required)
- Current CPR/BLS certification
- Valid Texas driver's license and reliable personal vehicle with current auto insurance
- 1+ year of clinical nursing experience (home health, med-surg, ICU, or similar)
- OASIS certification or experience a strong plus (RN applicants)
- Excellent communication skills and a genuine passion for patient-centered care
- Ability to work independently, manage your schedule, and prioritize patient needs
- Must pass background check and drug screening
