Our client is seeking a dedicated case manager for summer support at their camp in Chesterfield, VA. Work with children from 6 years of age through adults who are participants in the camp.

Full schedule, 8:30 - 4:30.

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Develop, update and monitor person-centered individual education plans (IEPs) or individual support plans (ISPs)
* Review and audit daily documentation
* Complete assessments as needed

CLIENT'S REQUIRED EXPERIENCE AND EDUCATION
* Bachelor's in Social Work, Psychology, Human Services, or related field
* Minimum of 2 years of experience working with persons with disabilities or medical concerns including experience in service management and related documentation.
* Solid understanding of person-centered planning and Medicaid Waiver documentation
