Full Time – M-F 9am-5pm some weekends

We seek a full time Assistant Director of Case Management play a key leadership role in overseeing the coordination, quality, and effectiveness of the Case Management Department. Working under the Director of Case Management, the position supports strategic planning, operational management, and staff development to ensure patients receive timely, patient-centered, and cost-effective care.

As an Assistant Director, you will provide the day-to-day supervision of nurse case managers and support staff--monitoring throughput, length of stay, discharge planning efficiency, utilization management, and interdisciplinary collaboration.

We require:                                                             

• BSN Required. MSN preferred
• Current and valid NYS licensure required.
• CCM preferred
• 5 years Clinical Experience in Case Management, Utilization, Discharge Planning

• Minimum of 4 years of direct experience in utilization management, discharge planning, case management or home care management required.
• Minimum of 2 years of leadership or managerial experience in a healthcare environment, preferred.
• Strong English written and verbal communication skills
• Project Management skills

Pay Range USD $148,000- USD $160,000/Yr.
